Host Homes


Community Works is offering emergency shelter to youth in the Medford area. The emergency shelter is part of Community Works Home Run program, serving homeless and runaway youth in Jackson County. Youth may be placed in a Host Home for up to 21 days and receive case management and therapy services.


Eligibility:

  • Between the ages of 11 and 18
  • Youth cannot be a ward of the court, have an open child welfare case, or have a probation officer

Goals of the program:

  • To provide case management and resource development to assist youth and their families with needed community services / resources.
  • To reunite families by providing support through counseling and mediation to youth and families using a strength and resolution-based model of care

Clients volunteer to:

  • Cooperate with the staff and host home family
  • Participate in case management and therapy sessions
  • Participate in school, work or other activities during daytime hours
  • Have parental consent to stay in the host home
